Woolston and Chartwell Partnership in Southampton seeks an enthusiastic Advanced Clinical Practitioner (ACP) to join a 6-partner practice supported by a full MDT including nurses, HCAs, pharmacists and social prescribers.
The ACP will work autonomously within scope, managing minor and acute care, participating in care planning, and contributing to same-day demand and urgent care needs across face-to-face, video, telephone, and home visits.
We are looking for an enthusiastic ACP to join our friendly, forward thinking team. We are a 6 partner practice with 8 regular GPs supported by our friendly group of Nurses and HCAs.
This position is open to Acute Nurse Practitioners / Paramedic Practitioners, or Advanced Clinical Practitioners.
The non-clinical team at our practice is highly skilled and very efficient, having a fluid working system with our clinicians. We also benefit from having a First Contact Physio, 2 Clinical Pharmacists, 2 Pharmacy Technicians, 3 Social Prescribers, 2 Health and Wellbeing Coaches, a Physician Associate and 2 Mental Health Workers on hand to help with the workload.
Beyond the expected assessment, treatment and collaboration as required, you will sometimes need to refer patients for investigations and/or the secondary care input, you will ensure the provision of continuity of care including follow‑up if required. You will contribute to the writing of Care Plans for those patients at risk of hospital admission where necessary.
You will operate within your own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence.

Woolston and Chartwell Partnership, a friendly practice with two sites located on the east side of Southampton. We have a large housebound and care home population who require domiciliary consulting.
We already have well established clinical pathways across our extensive MDT, so you would be supported by colleagues of different professions including MH Practitioners, social prescribers, pharmacists, as well as experienced GP's and existing ANP/Paramedics.
The ACP will act autonomously within their professional scope of practice providing care for patients presenting with minor and acute illness and injury, working with the GPs to support same day demand and acute care needs of the patients. This will be via face to face, video, telephone and e‑consultations and home visits.
The ACP will play a key role in improving access to healthcare services, supporting long‑term condition management, and promoting high standards of clinical care.
