Job summary

Greenfields Medical Practice in Nottingham is seeking an Advanced Nurse/Clinical Practitioner to join a friendly multidisciplinary team. You will provide autonomous patient-centred care, support prescribing and annual reviews, and liaise with external services to aid vulnerable patients.

You will work with GPs, nurses, pharmacists and admin staff in a well-established GP practice. The role emphasises professional development and adherence to local policies, with strong safeguarding and auditing

Qualifications

We are looking for an enthusiastic and motivated Advanced Nurse/Clinical Practitioner to join our friendly, supportive, and experienced multidisciplinary team. The following are the core responsibilities: Extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required Contribute to practice targets (QOF etc.), complying with local and regional guidance Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)Support the clinical team with all safeguarding matters, in accordance with local and national policies IPC training to be completed as part of the mandatory training and induction. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Deliver opportunistic health promotion where appropriate Ensure accurate and legible notes of all consultations and treatments are recorded in the patients care record Ensure collection and maintenance of statistical information required for regular adhoc reports and audits Attend and participate in Practice Meeting as required Undertake home visits to our housebound patient population for the purpose of assessment and treatment for acute or chronic conditions Ensuring when significant events are identified, they are recorded appropriately on GP Team Net., To work as an autonomous practitioner, responsible for the delivery of practice care services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the patient population of the Greenfields Medical Practice . The advanced nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.The following are the core responsibilities of the Advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels: Develop, implement and embed health promotion and well-being programmes Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients Implement and evaluate individual specialised treatment plans for chronic disease patients Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF Undertake the collection of pathological specimens Request pathology services as necessary Process and interpret pathology and other test results as required Maintain accurate clinical records in conjunction with extant legislation using primary care software Ensure read codes are used effectively Maintain chronic disease registers Prioritise health issues and intervene appropriately Support the team in dealing with clinical emergencies Recognise, assess and refer patients presenting with mental health needs Extended and supplementary prescriber, adhering to extant guidance Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required Contribute to practice targets ,QOF etc., complying with local and regional guidance Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.) Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) Support the clinical team with all safeguarding matters, in accordance with local and national policies IPC training to be completed as part of the mandatory training and induction.
