Cambridgeshire County Council is seeking an experienced Adult Support Co-ordinator for the South Transfer of Care Team, covering Addenbrooke’s Hospital and surrounding areas. The role focuses on planning safe discharges, arranging appropriate care, and conducting six-week post-discharge reviews to ensure positive outcomes.

Qualifications

• Experience in health or social care settings.
• Experience with Care Act Assessments or similar discharges.
• Ability to travel across the county to multiple locations.

Responsibilities

• Carry out Care Act Assessments for adults preparing to leave hospital or rehabilitation settings.
• Identify care and support needs and arrange timely provision for safe discharges.
• Complete six-week post-discharge reviews to ensure ongoing needs are met.
• Collaborate with therapists, nursing staff and care providers to deliver person-centred support.
