Job summary

Cambridgeshire County Council seeks an experienced Adult Support Co-ordinator to join the South Transfer of Care Team. You will support adults 18+ to plan safe discharges from hospital, arrange care, and complete post-discharge reviews, working with therapists, nursing staff and care providers to secure timely outcomes.

Based across Addenbrooke’s Hospital and community settings, you’ll carry a busy caseload, travel across the county as required, and participate in safeguarding activities.

Qualifications

• Experience working with adults in a health or social care setting.
• Experience in statutory social services is desirable.
• Able to travel independently across the county and work from multiple locations.

Responsibilities

• Support adults aged 18+ to plan safe hospital discharges.
• Arrange appropriate care and support and complete six-week post-discharge reviews.
• Contribute to safeguarding enquiries as part of the Adult Support Co-ordinator role.
• Collaborate with therapists, nursing staff and care providers to deliver person-centred support.
