Job summary

Hertfordshire Partnership University is seeking a Discharge Coordinator to support NHS discharge processes across Hertfordshire. You will work within an Enhanced Discharge Team to facilitate timely discharges from inpatient wards back to the community.

Working across Adult Acute Inpatient wards and contracted providers, you will collaborate with inpatient staff, housing workers and social care teams to remove barriers to discharge and promote patient wellbeing.

Responsibilities

• Work as part of an MDT within Enhanced Discharge Team to support discharge from inpatient wards to communities or signpost to CSPS and rehab services.
• Work across Adult Acute Inpatient wards in Hertfordshire and contracted independent sector providers.
• Collaborate with inpatient staff, medical team, CATT, bed management, housing worker, health and social care teams, Bed Placement and carers to identify and resolve discharge barriers promptly.
