Senior Assistant Practitioner and Coordinator

Coordinate for the Transfer of Care Service, supporting hospital discharges, Urgent Care Response and Virtual Wards but ensuring clear communication and coordination of the service. Support in times of high pressures, reprioritising workloads of self and others to ensure continuous and smooth processes. Maintain a strong and professional relationships with systems partners, working in a collaborative, support and professional manner. Maintains own caseload of clients for rehabilitation and self management of conditions, providing education and health promotion to the population of Bedfordshire. To support management in times of service pressures. To have own vehicle to travel the County of Bedfordshire.
