Job summary

Buckinghamshire Healthcare NHS Trust is seeking a Non-Clinical Discharge Co-ordinator to support the discharge team, coordinating care for patients with complex discharge needs from admission to safe discharge across Buckinghamshire hospital sites.

You will liaise with ward staff, social services, carers and communities; strong interpersonal and communication skills, time management, and the ability to work independently and as part of a team are essential.

Qualifications

• Excellent interpersonal and communication skills
• Good time management skills
• Ability to prioritise and work on own initiative as well as part of a team
• Experience supporting discharge planning for complex needs

Responsibilities

• Support the delivery of high quality discharge and patient flow for patients with complex discharge planning needs.
• Liaise with ward staff to identify patients requiring intervention and coordinate assessments/referrals.
• Communicate with patients, relatives and carers and with community, social services and other agencies to develop an optimal discharge plan
