Job summary

CMHA-WECB is seeking a Supportive Housing Case Manager-Youth to help establish and maintain safe, stable housing for youth with serious mental illness. You will do outreach, build therapeutic relationships, develop care plans and provide crisis intervention within a client-directed framework.

The role emphasizes wellness, independence and recovery, working with a housing team and landlords to secure market-based housing and supportive services for youth aged 15-30.

Qualifications

  • Bachelor level degree in social work, nursing or relevant field preferred.
  • Registered member in good standing of a relevant professional college is an asset.
  • Experience with vulnerable populations and youth housing is an asset.

Responsibilities

  • Support establishment and maintenance of safe, affordable housing for youth with mental illness.
  • Provide client directed case management, outreach, and care planning.
  • Deliver education on illness, symptom management and life skills.
  • Collaborate with landlords and community partners to sustain market housing.
  • Conduct relapse prevention, crisis intervention and advocacy services.
  • Maintain thorough documentation and participate in program evaluation.
