Job summary

Bayshore HealthCare in Hamilton, ON is seeking a Client Service Coordinator for weekends to connect patients, families and healthcare providers with compassionate care planning.

You will schedule care providers, manage funder referrals in Procura EMR, and ensure accurate records while coordinating with supervisors and the broader team to meet client needs in a fast-paced home care setting.

Qualifications

• Minimum two years of related customer service experience, preferably in healthcare.
• Proven customer service and communication skills in high-paced environments.
• Excellent computer skills with EMR access and MS Office.

Responsibilities

• Engage professionally with patients, families, staff, and funders.
• Process funder referrals and EMR updates quickly (Procura).
• Collaborate with supervisors, staff, and patients to assess service needs.
• Schedule care providers based on client needs and staff availability.
• Maintain accurate records in the EMR and notify changes to stakeholders.
