Job summary

Bayshore HealthCare is a leading provider of home and community health care services in Canada. We are seeking a Client Scheduling Coordinator to link patients, families, and healthcare providers, manage referrals in Procura, and assign qualified caregivers based on client needs and geography.

The role requires at least two years of healthcare customer service experience, strong English communication, and proficiency with EMR systems and scheduling tools.

Qualifications

• Minimum of two years related customer service experience, preferably in healthcare.
• Strong written and verbal English communication skills.
• Ability to handle multiple demands in a fast-paced environment.
• Proficiency with EMR systems and scheduling software.
• Excellent computer skills (Windows, Microsoft Office).

Responsibilities

• Communicate professionally with patients, families, staff, and funder organizations.
• Process funder referrals and updates in the EMR (Procura).
• Schedule care providers based on client needs, geographic area, staff skill-set and availability.
• Coordinate caregiver assignments and service delivery.
• Document accurate records in the EMR and keep patients informed of initial and ongoing schedules.
• Resolve patient concerns promptly and follow escalation pathways.
