Job summary

Qualifications

• Education includes a secondary diploma and related certificates preferred.
• Minimum two years of customer service experience, preferably in healthcare.
• Ability to communicate effectively in English, both written and oral.

Responsibilities

• Communicate professionally with patients, families, staff and funder organizations.
• Process funder referrals and updates in the EMR (Procura).
• Review service needs with supervisors, staff and patients.
• Schedule care providers based on client needs and location.
• Coordinate caregiver assignments and service delivery.
• Notify patients and staff of initial and ongoing schedules.
• Document records accurately in the EMR and resolve issues promptly.
