Job summary

Responsibilities

Engage professionally with patients, families, staff, and funder organizations.
Process funder referrals and updates in the EMR (Procura).
Collaborate with Supervisors, field staff, and patients to assess service needs.
Assign care providers based on client needs, location, and staff availability.
Coordinate consistent caregiver schedules and service delivery.
Keep patients and staff informed of initial and ongoing schedules.
Document accurate records in the EMR and resolve patient concerns promptly.
