Job summary

West Coast health employer Vancouver Coastal Health seeks a Social Work 2 (Case Manager) in Vancouver to deliver client-centred case management and clinical social work services for individuals with complex mental health and addiction needs.

The role includes conducting biopsychosocial assessments, counselling, and coordinating care with interdisciplinary teams while ensuring continuity of care and crisis intervention.

Qualifications

• Master's degree in Social Work required; BC College of Social Workers registration preferred.
• Minimum of 2 years' recent case management experience with complex mental health/addiction cases.
• Valid BC Driver's License.
• Current full registration with BC Social Workers required/preferred.

Responsibilities

• Provide case management and clinical social work services within a recovery-focused model.
• Conduct biopsychosocial assessments and provide counselling.
• Maintain client records and coordinate care across interdisciplinary teams.
• Deliver crisis intervention and plan transitions to community supports.
• Act as the Designated Responder under applicable acts when appropriate.
