Job description

Come work as a Team Leader, Primary Care (Social Worker) with Vancouver Coastal Health (VCH) on the Primary Care Support team in North Vancouver, BC. Within the context of a client and family-centred framework and continuum based care model and in accordance with established standards of professional practice and the vision and values of the organization, including Indigenous Cultural Safety, cultural humility and anti-racism principles, the Team Leader, Primary Care (Social Work) oversees and coordinates the delivery/daily activities of designated programs/areas. Evaluates services to ensure program/service needs are met and liaises with key partners on the achievement of program goals and objectives. Promotes interdisciplinary practice and fosters a safe environment for staff and clients. Provides leadership for the delivery of a service, working in partnership with the organizational resource areas, including Professional Practice, Clinical Education, Clinical Quality and Safety, and other organizational key partners, to ensure a high standard of care founded in best practice and standards of care. Provides supervision to staff which may include Concurrent Disorders Clinicians, Clinical Counselors, Care Coordinators and/or support staff such as Community Liaison Workers and Clinical Support Attendants. Provides continuous communications, conflict resolution, performance recognition, coaching, performance management, safe work environment, facilitates client transitions, and manages workload and staffing issues. Supports the Manager with programs related to attendance wellness promotion, respectful workplace, accommodation, recruitment, hiring, selecting, mentoring and retaining staff. Maintains a clinical caseload, plans and implements social work services by conducting social and psychosocial assessments; participating with clients, family, community support and interdisciplinary team in case management meetings and maintaining related client records. The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes. As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. For all new hires and appointments to Vancouver Coastal Health, you will be asked to provide this information as part of the onboarding process.

Education & Experience

• Current full registration with the British Columbia College of Social Workers.
• Master of Social Work from an accredited School of Social Work.
• Five (5) years recent related clinical experience working with individuals with mental health and/or primary care needs including two (2) years supervisory and/or leadership experience or equivalent combination of education, training and experience.
• Valid B.C. Driver's license and local area travel requiring the use of a vehicle may be required.

Knowledge, Skills & Abilities

• Broad knowledge of systems thinking as it related to social work process.
• Broad knowledge of other health disciplines and their role in healthcare.
• Broad knowledge of case management and practice within a client and family centred care model for clients with complex health care needs.
• Knowledge of appropriate acts; e.g. Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.
• Broad knowledge of counselling skills and clinical practice models.
• Demonstrated ability to provide competent and culturally safe care in a variety of settings and with diverse populations.
• Knowledge of cultural diversity in youth and families, and culturally safe ways of engaging.
• Demonstrated ability to apply systems theory to practice in Social Work assessment and intervention.
• Demonstrated ability to conduct social and psychosocial assessments.
• Demonstrated ability to understand and influence group and inter-group processes, decision-making and problem solving.
• Demonstrated ability to establish rapport with clients and families in order to explore issues that are beyond the initial presented concerns.
