Job summary

Vancouver Coastal Health in Whistler, BC is seeking a Mental Health & Addictions Case Coordinator Nurse (RN or RPN) to join the Adult Mental Health team. You will work within a psychosocial rehabilitation model to provide treatment and case management services, coordinating care with families and colleagues.

You will conduct intake assessments, plan treatment, and deliver counseling and education to support client access to services.

Qualifications

• Diploma in Nursing or Psychiatric Nursing coursework or equivalent.
• Current RN or RPN registration with the BC College of Nurses and Midwives.
• Minimum of 2 years’ recent experience in mental health or addictions.
• Valid BC driver’s license; local travel may be required.

Responsibilities

• Conduct intake and mental health assessments.
• Develop and manage individualized treatment plans.
• Provide crisis intervention and outreach as needed.
• Deliver individual counseling, education, and support groups.
• Coordinate access to services across the care continuum within a multidisciplinary team.
