Qualifications

  • Valid Driver’s License and reliable car are required.

Responsibilities

  • Provide nursing care in the community focusing on peritoneal dialysis and wound care.
  • Conduct patient and family teaching.
  • Chart patient assessments and communications effectively.

Core Competencies:
  • Registered with the College of Nurses of Ontario (CNO) and in good standing
  • Experienced in Home and Community Care is an asset
  • Current BCLS certification (Level C)
  • Demonstrated clinical assessment and patient/family teaching skills
  • Demonstrated effective communication skills
  • Charting on Procura is an asset
Requirements:
  • Clear & updated Vulnerable Sector Check (within last six months)
  • Valid Driver’s License & reliable car in community homecare is required
  • Up to date TB test within last 2 years
  • Must have your own: a) Stethoscope b) Blood pressure cuff and Sphygmomanometer

Shifts: 2 or 3 days in a week and alternate weekends

Note: This is a specialty program for Peritoneal Dialysis, Wound Care, IV Management, Diabetic Care & Post Operative Care in the community.
