Job summary

AmerisourceBergen is seeking a Field Case Manager who can operate from a home office with periodic in-person visits to clinics. You will guide patients, physicians, and program partners throughout the therapy journey, ensuring coordinated, patient-centered care while navigating reimbursement and enrollment processes.

The role emphasizes strong communication, organization, and the ability to work remotely with occasional travel.

Qualifications

• RN or related degree required or equivalent transferable skills.
• Knowledge of public or private third-party reimbursement is a plus.
• Bilingualism is an asset (Quebec requirement).
• Experience as a Case Manager is an asset.

Responsibilities

• Serves as the primary contact for physicians and program stakeholders.
• Educates providers on services, patient support resources, reimbursement, and enrollment.
• Reviews patient charts and ensures completion of required documentation.
• Owns the special authorization process for timely submissions and guidance on reimbursement.
• Maintains ongoing communication with physicians about patient status and program changes.
• Adapts program elements to meet provider needs and supports in-clinic and remote care.
• Works autonomously in a remote setting and collaborates with team members.
