Job summary

United States Digital Space LLC is seeking a compassionate, patient-focused Part-Time Nurse Case Manager to support patients, healthcare providers, and clinic partners through a specialized Patient Support Program (PSP).

You will educate patients about therapies, coordinate care with physicians and clinics, and help patients navigate access to treatment and reimbursement while delivering an exceptional patient experience.

Qualifications

• Active nursing license in Alberta and/or British Columbia (RN, RPN/LPN, or equivalent).
• Diploma, degree, or equivalent education per P1 framework.
• Strong verbal and written communication skills.
• Proficiency using Microsoft Office and electronic documentation systems.
• Ability to travel to clinics across Alberta and British Columbia.
• Excellent organizational skills and attention to detail.

Responsibilities

• Serve as primary contact for patients in the PSP.
• Educate on therapies, expectations, administration, and adherence.
• Assist patient enrollment and onboarding.
• Coordinate care with physicians, clinics, and pharmacies.
• Navigate access to treatment and reimbursement processes.
• Perform patient assessments and follow-ups as required.
• Monitor progress and provide ongoing support.
• Document accurately in program systems.
• Elevate clinical or operational concerns as appropriate.
• Contribute to quality improvement initiatives.
• Build strong relationships with healthcare providers.
