Job summary

Cencora is seeking a Field Case Manager to guide patients, healthcare providers and program partners from a home office with in-person visits as needed. You will coordinate every step of the patient journey, advocate for patients, and address program changes while maintaining strong relationships with providers.

The role emphasizes remote work, bilingual English/French capabilities, and flexible, stable employment within a patient-centered health program.

Qualifications

• RN or related degree or equivalent transferable skills.
• Knowledge of public or private reimbursement arena.
• Ability to work autonomously in a remote setting; bilingual English/French essential in QC/NB.

Responsibilities

• Serve as primary contact for physicians in designated area.
• Educate providers on program services and patient resources.
• Review patient charts and ensure documentation is complete.
• Own the special authorization process for reimbursement.
• Maintain ongoing communication with physicians about patient status.
