Job summary

Carewell is seeking a Care Concierge to support members through enrollment, welcome visits and ongoing care navigation. You will coordinate with providers, pharmacies, and community resources while ensuring compliance and patient-centered communication.

The role is remote, Alberta-based, with a Monday–Friday schedule and a focus on building trust, explaining services, and guiding members through their early journey.

Qualifications

• 1+ year of member-facing healthcare experience.
• Ability to work Monday–Friday, 9am–6pm EST.
• Located in Alberta, Canada and authorized to work.

Responsibilities

• Enroll new program participants and secure participation.
• Conduct welcome visits and set expectations for ongoing coordination.
• Coordinate care navigation, bridging gaps with providers, pharmacies and community resources.
• Document interactions and maintain compliance throughout the process.
