Job summary

Calgary West Central Primary Care Network is seeking a Patient Care Coordinator to support physicians and teams by coordinating patient care to improve outcomes. You’ll work remotely using EMR systems and digital health platforms, with occasional in-person work at the Calgary area main office or clinics.

This role requires MOA certification or related credential, plus a minimum of two years in administrative roles within an interprofessional care team, and experience with medical records

Qualifications

• Two years of strong administrative experience in an interprofessional care team.
• Knowledge and experience with medical records management.
• Ability to maintain filing systems and basic databases.

Responsibilities

• Support routine panel maintenance and patient validation processes.
• Identify missing vitals, labs, screenings, and referrals.
• Ensure results of tests/procedures are available to providers (Alberta Netcare).
• Create and maintain notes on projects (minutes, ad hoc lists).
• Reach out to patients identified in projects via phone, email or text.
• Support clinicians with scheduling using EMRs and digital health platforms.
• Coordinate patient care with clinicians, including referral processing and informing patients.
