Job summary

VON Canada is seeking a licensed Registered Practical Nurse to provide holistic, person-centred home and community care in the Woodstock area. You will assess clients, deliver primary care in collaboration with other health professionals, administer medications, and support patients and families in managing chronic conditions and Palliative Care.

Work involves home visits, clinics, virtual care, and community outreach with a flexible schedule including days, evenings and weekends.

Qualifications

• Must be licensed as a Registered Practical Nurse in the province of practice.
• Community health experience is an asset.
• Must enjoy driving and have a valid driver’s license and access to a reliable vehicle.

Sakura House Residential Hospice provides specialized care and services with a patient-centered approach. Patients and families are intimately involved in creating their own plan of care which best addresses their unique needs. Our specialized hospice team works together, with other community partners and service providers to provide full support and quality hospice palliative care. Working within an interdisciplinary care team with the goal to provide comfort care and the management of pain and other symptoms.

• Must be able to wear Personal Protective Equipment (PPE)
