Job summary

ParaMed, Canada’s leading home health care provider, is hiring a Registered Nurse for community-based visits. You will assess patient needs, implement and monitor care plans, and build trusted relationships with patients in their homes.

Qualifications

• Must be registered with the CNO as a Registered Nurse in good standing.
• Valid driver's license and access to a vehicle are required.
• Experience delivering community care is an asset.
• Reliable and flexible with the ability to travel between patient residences.
• Willingness to provide on-call coverage, including after hours.

• Use appropriate assessment tools to assess individualized patient needs.
• Execute and continuously monitor care plans.
• Engage patients using a collaborative approach, building rapport and trust.

In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.
