Job summary

ParaMed Home Health Care is seeking a Registered Nurse to provide community nursing with scheduled visits to patients in their homes across Ontario. You will assess needs, execute care plans, and build rapport with patients to support comfortable, independent living.

The role offers autonomy, a supportive team, and opportunities for long-term relationships with patients, along with a strong clinical orientation and ongoing development.

Qualifications

• Registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s license and access to a vehicle.
• Experience in delivering community care is an asset.
• Willingness to travel to and from patient residences within a designated area.
• Required on-call coverage, including after hours to ensure patient safety and to meet patient care needs.
• Reliability and flexibility.
• Passion for making a difference in your community.

• Executing and continuously monitor care plans.

In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.
