Job summary

ParaMed Home Health Care is seeking a Registered Nurse to provide in-home visits across a community, caring for patients of all ages with varying needs. You will assess needs, implement and monitor care plans, and maintain patient records in partnership with families and healthcare teams.

The role offers autonomy, ongoing training, and opportunities for advancement, with full-time and part-time schedules including occasional weekends. A valid driver’s license and reliable vehicle are required.

• Active registration with the CNO as a Registered Nurse in good standing.
• Valid driver's license and access to a vehicle.
• Experience in delivering community care is an asset.
• Reliability and flexibility; willing to travel within a designated area.

In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What You’ll Do

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitor care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans

What You Bring
