Job summary

ParaMed Home Health Care is hiring a Registered Nurse for community-based home visits across Canada. You will assess patients, implement and monitor individualized care plans, and collaborate with families to promote comfort and independence.

A valid CNO RN registration and a driver’s license are required, with travel to patient residences within a designated area. The role emphasizes autonomy, ongoing clinical orientation, and opportunities for career development within a supportive team

In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What You’ll Do

• Using appropriate assessment tools/techniques to assess patients’ individualized needs
• Executing and continuously monitor care plans
• Engaging patients using a collaborative approach by building rapport and trust
• Maintaining and update professional patient records related to patient encounters and plans
