Job summary

ParaMed Home Health Care is seeking a part-time Registered Nurse to perform home visits in Fort Frances, Ontario. You will assess patients, implement and monitor care plans, and collaborate with families to ensure comfort and safety at home.

As a key member of the community care team, you will travel to patient residences, maintain records, and provide compassionate, patient-centered care with autonomy and support from a skilled nursing team.

Qualifications

  • Registered Nurse (RN) with CNO in good standing.
  • Valid driver's license and access to a vehicle.
  • Experience delivering community care is an asset.
  • Reliable and flexible with patient-focused approach.
  • Willingness to travel to patient residences within a designated area.
  • On-call coverage including after hours for patient safety.

Responsibilities

  • Assess patients' individualized needs using appropriate tools and techniques.
  • Execute and monitor care plans.
  • Engage patients with a collaborative approach to build rapport and trust.
  • Maintain and update professional patient records.

This vacancy is located in Fort Frances, Ontario.

In this community nursing role, as a Registered Nurse (RN) you will have scheduled visits to care for patients in various settings. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What You’ll Do
  • Using appropriate assessment tools/techniques to assess patients’ individualized needs
