The Registered Nurse IIC (Care Facilitator) is a registered nurse who leads in coordinating daily activities of the chemotherapy and outpatient clinics within the Cancer Care Program – Central at Central Health Centre in Grand Falls-Windsor.

The role emphasizes continuity of patient care, clinical judgment, and participation in quality and patient safety initiatives, ensuring high standards of care and effective team collaboration with other disciplines.

Formación

• Graduation with a Baccalaureate degree in Nursing or equivalent.
• Current and continuing practicing RN licensure with the Newfoundland and Labrador College of Nurses.
• Minimum of five years' experience within the last seven in adult Oncology.
• CONC certification.
• Current BLS Certification.

Responsabilidades

• Coordinate daily activities of the chemotherapy and outpatient clinics.
• Ensure continuity and coordination of patient care using clinical judgment.
• Contribute to quality and patient safety initiatives and evaluate outcomes.

Job Summary

The Registered Nurse IIC (Care Facilitator) is a registered nurse who functions in a leadership capacity, coordinating the daily activities of the chemotherapy and outpatient clinics. The Care Facilitator will be accountable for the continuity and coordination of patient care utilizing clinical judgment and critical thinking skills; assist with the development, implementation and evaluation of quality and patient safety initiatives; and ensure a high standard of patient care is maintained. The Care Facilitator will evaluate the effectiveness of patient care through such mechanisms as team conferences, ongoing feedback with team members/patients/family, and review of staff assignments. The Care Facilitator also participates as an active team member and acts as a liaison for team members in the interdisciplinary provision of care; facilitates and coordinates patient/family care with other disciplines, departments and programs; and performs other related duties as required.

Job Qualifications

• Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing or equivalent education or training is required.
• Current and continuing practicing licensure as a Registered Nurse with the Newfoundland and Labrador College of Nurses.
• Minimum of five (5) years' experience within the last seven (7) in adult Oncology is required.
• CONC certification
• Current BLS Certification is required.
• Demonstrated competency in organizational, leadership, interpersonal, team-building, communication, teaching and decision-making skills.
• Demonstrated initiative and ability to work independently.
• Demonstrated progressive leadership.
• Evidence of active involvement or willingness to participate in continuing professional education development.
• Membership in professional organizations/special interest groups is an asset.
• Knowledge of continuous quality improvement concepts.
• A satisfactory record of work performance and attendance is required.
