Job summary
Carefirst Family Health Team (FHT) seeks a Quality Improvement Decision Support Specialist to join our interdisciplinary team delivering high-quality community-based care in Toronto. This full-time permanent role involves data analysis from EMR and operations, building dashboards, and partnering with leadership to guide program decisions.
The ideal candidate has a post-secondary degree in data science or related field and at least two years of data analytics experience, with familiarity in PHI
Qualifications
- Post-secondary degree or diploma in Data Science, Health Science, Data Analytics, Statistics, Epidemiology, Information Technology, Computer Science, or a related discipline.
- Two+ years of data analysis, AI, health informatics, or similar experience; primary care or community services is an asset.
- Strong data quality and reporting skills; experience with EMR-based reporting is an asset.
Responsibilities
- Gather, clean/review, and analyze data from EMR, operational, and administrative systems to support quality improvement and decision-making.
- Guide clinical decisions on patient-oriented programs and improvement initiatives.
- Develop reports, dashboards, and data visualizations for clinical and non-technical audiences.
