Job summary

CBI Home Health is seeking a Physiotherapist to support patients in the East of Ottawa region via home-based, integrated care. You will help clients rebuild strength and function after hospitalization, promoting independence and safety in daily life.

As a PT in our Integrated Care Program, you will travel between homes, collaborate with an interdisciplinary team, and contribute to reducing readmissions while delivering compassionate, patient-centered care.

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Ability to travel within the community; valid driver's license, vehicle and car insurance required.
• Commitment to person- and family-centered care and collaborative practice.

Responsibilities

• Help patients rebuild strength and function after hospitalization in a community setting.
• Provide hands-on, integrated care to reduce readmissions and improve outcomes.

Compensation Range: The salary range for this position is $58.75 – $67.40 per visit.

What you will need to be successful

• Registered in good standing with the College of Physiotherapists of Ontario

• Strong communication skills and the ability to collaborate within an interdisciplinary team while working independently
