Job summary

CBI Home Health is seeking a Physiotherapist to deliver hands-on, community-based therapy through our Integrated Care Program in the Toronto/North York area. You will help patients rebuild strength and function after hospitalization, working within a team that values person-centered care and community partnerships.

You will travel between patient homes across the Toronto region, coordinating with an interdisciplinary team to reduce readmissions and support patient goals with evidence-based

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Ability to travel from home to home within the community; valid driver’s license and vehicle required.

Responsibilities

• Provide physiotherapy services in patients' homes within the Toronto/North York region.
• Help patients rebuild strength and function to improve outcomes.
• Collaborate with an interdisciplinary team in an Integrated Care Program.

Salary Range: The salary range for this position is $59.93 – $69.04 per visit.

What You Will Need To Be Successful

• Registered in good standing with the College of Physiotherapists of Ontario
• Ability to travel from home to home within the community- a valid driver’s license, vehicle and car insurance is required
• A commitment to person and family-centered care
• Passion, empathy, and critical thinking in client decision-making
• Strong communication skills and the ability to collaborate within an interdisciplinary team while working independently
• A deep appreciation for the role of a therapist in keeping clients at home when it aligns with their personal goals
