Job summary

CBI Home Health in Ontario seeks an Occupational Therapist to support clients in the Huron Perth region. You will assess, plan, and deliver OT interventions in home and community settings, focusing on enabling independence and safe self-care.

Qualifications

• Must be a registered OT in good standing with the College of Occupational Therapists of Ontario.
• Must be able to travel within the community with valid driver's license and insurance.
• Demonstrates person- and family-centered care with empathy and strong communication.
• Exhibits sound judgment and critical thinking in client care decisions.

Responsibilities

• Assess and implement OT interventions, ADL/IADL strategies, equipment needs, and home modifications.
• Collaborate with an interdisciplinary care team to deliver services.
• Provide holistic care including health coaching and self-management support.
• Utilize digital platforms for care delivery, documentation, and intervention.
