Job summary

Qualifications

• Baccalaureate degree in Nursing or equivalent education/experience.
• Master's degree in health studies preferred.
• Current, unrestricted RN registration with the College of Nurses of Ontario.
• Two years hospital experience with intake/discharge planning preferred.
• Experience with electronic patient record systems.

Responsibilities

• Lead bed utilization and other key meetings to coordinate patient flow.
• Review daily dashboard data and ensure data quality.
• Provide summaries to Providence Care leadership on admissions and discharges.
• Coordinate transfers and assess admission decisions for patient flow.
• Support safe, person-centered discharges across care settings.
