Job summary

Shared Health at Health Sciences Centre Winnipeg is seeking an experienced nurse to coordinate patient movement across admission, transfer and discharge within Adult Medicine/Family Medicine and ER programs. The role requires strong liaison with multidisciplinary teams and external agencies.

The successful candidate will have a Bachelor of Nursing, active CRNM registration, and at least four years of recent nursing experience, with CPR training in line with policy.

Qualifications

• RHPA scope of practice and professional standards required.
• Proficient with Microsoft Word and Excel.
• Experience coordinating patient admission and discharge across teams.
• Strong interpersonal and communication skills; Indigenous language skills preferred.

Responsibilities

• Coordinate patient movement through admission, transfer and discharge.
• Liaise between programs and external agencies on patient movement.
• Develop guidelines and educational strategies for patient movement.
• Contribute to site resource optimization and admissions decisions.
