Job summary

AgeCare is seeking a Registered Practical Nurse (RPN) to provide professional nursing care to residents in a Don Mills, Toronto setting. This part-time role will involve directing Personal Support Workers and coordinating with physicians and the health care team.

You will work under the guidance of a Registered Nurse to ensure residents receive high-quality care in a supportive, home-like community.

Qualifications

A sound knowledge and background in geriatric, rehabilitation or long-term care nursing.
Familiarity with the Fixing Long-Term Care Act and Regulations.
Knowledge of Ontario health acts and safety acts (Health Disciplines Act, OHS Act, Pharmacy Act, Food and Drugs Act, Controlled Substances Act).
Current Certificate of Competence from the College of Nurses of Ontario.

Responsibilities

In collaboration with the Registered Nurse, implements AgeCare policies and procedures on the unit.
Liaises with physicians and health care team members and notifies physicians of significant changes when the RN is unavailable.
Works with care team members to ensure residents’ care needs are met.
Provides direction to Personal Support Workers on the care team.
Participates in shift report to inform team of resident status changes.
