Unison Health and Community Services in Toronto seeks a Part-time Case Coordinator to join the Primary Health Care team. You will handle intake, assess determinants of health, and develop plans of action to meet client needs.

The role involves advocacy, short-term counselling on housing, immigration, income security, and food security, plus collaboration with community groups and service providers. Flexible hours and a commitment to inclusive, harm-reduction principles are expected.

Formación

• Undergraduate degree in social services or related disciplines with prior health or social service experience.
• Post secondary degree or diploma in social services or related disciplines with prior experience.
• Knowledge of health and social service agencies and resources.
• Skills in supportive short-term counselling.
• Experience with group work and community development.
• 3–5 years in a community-based setting.
• Experience with Electronic Health Records (EHR) and PSS is an asset.
• Understanding of local issues in the geographic area.

Responsabilidades

• Provide client intake and orientation to new clients.
• Assess the impact of determinants of health on wellbeing and develop a plan of care.
• Advocacy and referrals to internal/external programs.
• Provide short-term counselling on housing, social assistance, immigration, income security, family support, and food security.
• Develop intervention plans for abuse-related issues.
• Share community resources with clients and organizations.
• Act as a resource for community groups and networks.
• Support advocacy related to community issues.
• Participate in Quality-of-Care audits across sites.
