Ontario Health atHome is seeking a Care Coordinator to assess patient needs, determine eligibility, and connect patients with community services. You will coordinate care plans with families and a multidisciplinary team, ensuring patient-centered delivery across home, community, or facility settings.

Required Ontario regulatory membership and 2+ years in community health, with strong communication, problem-solving, and time-management skills.

Qualificações

• Must be in good standing with Ontario regulatory bodies.
• 2+ years of community health or related field experience.
• Knowledge of health care delivery systems and community resources.
• Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.
• Strong time management and ability to work independently in multidisciplinary teams.

Responsabilidades

• Assess care needs and determine eligibility for services.
• Link patients with service providers and coordinate care plans.
• Monitor and coordinate delivery of care as planned.
• Establish rapport with patients and families and respect preferences.
• Complete required documentation accurately and on time.
