Job summary

CBI Home Health in Hamilton is seeking a Physiotherapist to deliver integrated care directly from hospital to home, helping clients recover, regain independence and achieve better outcomes at home.

You will work with hospital and community teams, manage clinical caseloads, and provide holistic, coordinated care with flexible scheduling and supportive resources.

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Ability to travel within the community with a valid driver’s license, vehicle and insurance.
• Commitment to person- and family-centered care.
• Strong communication and teamwork in an interdisciplinary setting.

Responsibilities

• Develop and implement individualized treatment plans to maintain and enhance physical abilities and well‑being.
• Manage and prioritize clinical caseloads; coordinate and deliver therapeutic services.
• Collaborate in a holistic care model to meet client needs.
• Foster positive relationships with clients, team members, funders and the community.

Salary Range: The salary range for this position is $74.52-$85.53

• Work with clients to develop and implement individualized treatment plans, directed towards maintaining and enhancing their physical abilities, lifestyle and well-being.
• Be responsible for managing and prioritizing clinical caseloads, and for the coordination and delivery of therapeutic services.
• Participate in a collaborative care model providing holistic, comprehensive care to meet the needs of clients.
• Promote positive relationships between clients, team members, funders and the community.

What you will need to be successful

• Registered in good standing with the College of Physiotherapists of Ontario
• Ability to travel from home to home within the community- a valid driver’s license, vehicle and car insurance is required
• A commitment to person and family-centered care
• Passion, empathy, and critical thinking in client decision-making
• Strong communication skills and the ability to collaborate within an interdisciplinary team while working independently
• A deep appreciation for the role of a therapist in keeping clients at home when it aligns with their personal goals
