Job summary

CBI Home Health is seeking an Occupational Therapist to support clients at home in Chatham-Kent. You will provide OT interventions, ADL/IADL strategies, equipment recommendations, and home modifications.

You’ll collaborate with an interdisciplinary team, deliver health coaching, and utilize digital platforms for care delivery and documentation. A flexible schedule and added benefits support work-life balance.

Qualifications

• Registered OT in Ontario in good standing.
• Valid driver’s license and insurance for community travel.
• Comfort with person- and family-centered care.
• Strong clinical judgment and critical thinking.
• Able to work both in teams and independently.

Responsibilities

• Assessment & Implementation: provide OT interventions, ADL/IADL strategies, equipment recommendations, and home modifications.
• Team Collaboration: work with interdisciplinary care teams.
• Holistic Care: restorative health approaches, health coaching, and self-management support.

• Professional Standing: You are a registered member in good standing with the College of Occupational Therapists of Ontario.
• Mobility: Ability to travel within the community - valid driver's license and insurance required
