Job summary

CBI Home Health is hiring an Occupational Therapist in Chatham-Kent, ON to provide home-based assessments and interventions. You will support ADL/IADL independence, recommend equipment, and modify homes to improve safety and function.

You will collaborate with a multidisciplinary team and use digital tools for care delivery and documentation. The role offers a flexible schedule, travel within the community, and opportunities for professional growth.

Qualifications

• Must be registered in good standing with the College of Occupational Therapists of Ontario.
• Ability to travel within the community with valid driver’s licence and insurance.
• Strong communication skills and patient-/family-centered care approach.
• Excellent problem-solving and clinical decision-making abilities.

Responsibilities

• Provide OT interventions and home modifications, ADL/IADL strategies, and equipment recommendations.
• Collaborate with a interdisciplinary care team to deliver services.
• Support restorative health approaches, health coaching, and self-management.

Salary Range: The salary range for this position is $82 – $86 per visit
