Job summary

Sentrex Health Solutions, a proudly Canadian specialty distributor and patient support provider, seeks a Nurse Case Manager to coordinate patient services and program support. You will liaise with HCPs, patients, insurers, and pharmacies to facilitate access and coverage for therapies.

Qualifications

• Must be a Registered Nurse in good standing with the provincial College.
• Bilingual English/French is preferred.
• 2–4 years of clinical experience.
• Experience with Patient Support Programs is a strong asset.
• Immunology experience is an asset.
• Bilingual in English and French is an asset.

Responsibilities

• Coordinate HCP and patient registration per program protocols.
• Handle data entry for program services at contact points.
• Answer general inquiries about program services and disease states.
• Provide medical support and disease state education to patients and HCPs.
• Participate in training sessions for therapies with pharmaceutical teams.
• Provide inbound/outbound telephone support for adherence and follow-up.
• Adhere to case-management protocols including monitoring and reporting.
