Job summary

CBI Home Health is seeking a Physiotherapist to support patients in the Bradford region, delivering integrated, community-based care that helps individuals regain strength and function at home.

You will travel between community homes, maintain registration with the Ontario College of Physiotherapists, and provide patient-centered care with strong communication within an interdisciplinary team.

Qualifications

• Registered in good standing with the College of Physiotherapists of Ontario.
• Valid driver’s license, vehicle and car insurance are required.
• Commitment to person and family-centered care.
• Passion, empathy, and critical thinking in client decision-making.
• Strong communication and collaboration within an interdisciplinary team.
• A deep appreciation for the role of a therapist in keeping clients at home when it aligns with their personal goals.

Responsibilities

• Bring mobility and recovery home through hands-on, community-based care.
• Help patients rebuild strength and function after hospitalization; reduce readmissions.
