Job summary

The Care Company is seeking a Physiotherapist to join its community rehabilitation team. This flexible, casual opportunity can be structured as either an employee or independent contractor.

You will manage your own community caseload while receiving support from an interdisciplinary team to deliver person-centered care in clients’ homes across the Central Region of Ontario. You will assess, diagnose, and implement evidence-based interventions to improve mobility, balance, and independence, with

Qualifications

• Registered with the College of Physiotherapists of Ontario (CPO) in good standing.
• Experience delivering home and community care, preferably with HCCSS or Ontario Health atHome.
• Ability to work independently in the community and coordinate with interdisciplinary teams.

Responsibilities

• Perform in-home physiotherapy assessments within referral timelines.
• Develop and implement evidence-based treatment plans to improve mobility, balance and independence.
• Provide follow-up visits including gait training, falls prevention, and rehabilitation.
• Educate clients and caregivers and develop home exercise programs.
• Coordinate with Occupational Therapists and other professionals for equipment and safety.
• Maintain accurate documentation and monitor client outcomes.
