Job summary

Ontario Shores Centre for MHS in Whitby, ON, is seeking a Case Worker to support the Central Intake team. You will assist client-centered intake, screening, and navigation across the mental health and addictions system to facilitate access to services.

The role emphasizes reviewing referrals, conducting brief screenings, linking clients to appropriate services, and coordinating care with physicians, families, and community partners in a multidisciplinary, often virtual, setting.

Qualifications

• Education: College diploma or Bachelor’s degree in Nursing, Social Work, Psychology, Health Sciences, Behavioural Sciences, or related field.
• Experience: 3–5 years in mental health and/or addictions, including intake, navigation, or case management.

Responsibilities

• Review incoming referrals and determine eligibility for Ontario Shores outpatient services.
• Conduct brief triage interviews to understand client needs and match to programs.
• Assist clients with pre-screening, consent forms, and enrollment into patient portals.
• Redirect referrals outside eligibility criteria to appropriate resources.
• Provide practical assistance for social determinants of health to remove barriers to care.
• Facilitate communication with primary care providers and partner agencies for continuity of care.
