Job summary

Fraser Health is seeking a Coordinator for the Intensive Support & Stabilization Team (ISST) to manage daily operations in a defined geographic area. You will coordinate staff, scheduling, and HR-related events, while guiding training and quality improvement initiatives to support 24-hour home care services.

The role requires a Bachelor’s degree in a health discipline and at least five years of related experience, with strong leadership, reporting, and conflict-resolution skills.

Qualifications

• Bachelor's Degree in a health related discipline plus five (5) years of recent related experience working in a home support or long term care setting, or an equivalent combination of education, training and experience.

Responsibilities

• Provides administrative coordination and oversight to ISST team within the assigned geographical area.
• Oversees ISST staff scheduling, clerical support, performance feedback and review, discipline, and staff selection.
• Manages non-clinical complaints and staff grievances in collaboration with Employee Experience teams and Manager, Clinical Operations and Program Services.
• Develops standardized training, orientation, and guidance for ISST operations.
• Coordinates quality improvement initiatives including process changes for workflow, policy, staffing levels, and technology use.
• Develops and monitors operational reports and provides statistics to inform quality improvement.
• Conducts audits for standards compliance and quality improvement of client care.
• Participates in meetings and supports projects as assigned.
