Job summary

Fraser Health Authority is seeking a Relief Part-Time Coordinator, Home Health Service Line, based in Surrey, BC. You will supervise the daily activities of the Home Health Service Line, provide clinical leadership, coordinate staffing, and support discharge planning to ensure high-quality patient care.

This role requires RN registration with BCCNM and supervisory experience, along with a valid BC driver's license.

Qualifications

• Current practicing registration as a RN with BCCNM.
• Three years of related clinical experience including supervisory experience.
• Valid BC Driver's License and access to vehicle.

Responsibilities

• Supervises nursing staff and coordinates daily clinical team activities.
• Leads discharge planning and ensures appropriate patient placement.
• Provides clinical consultation on complex cases and supports staff with best practices.
• Liaises with referral sources to ensure smooth transitions to Home Health.
