Job summary

Fraser Health is seeking a Relief Part-Time Coordinator, Home Health Service Line in Surrey, BC. This role oversees daily operations, staff supervision, and discharge planning to ensure high-quality home health services.

Requires current BC RN license and supervisory experience in community care. The position involves coordinating clinical leadership, facilitating transitions from hospital to home, and collaborating with care teams to optimize patient outcomes.

Qualifications

  • Current RN registration with BC College of Nurses and Midwives (BCCNM).
  • Three years of recent clinical experience in community access and client assessment, plus one year of supervisory experience.

Responsibilities

  • Supervise nursing staff and coordinate day-to-day work; manage schedules, overtime, vacations and leaves to meet staffing requirements.
  • Lead discharge planning and ensure optimal patient placement and smooth transitions from hospital to home health.
  • Provide clinical consultation on complex cases and support Home Health Service Line operations.
  • Liaise with referral sources to ensure smooth transitions to Home Health from hospital or community.
  • Promote interprofessional collaboration and guide staff in using standardized assessment tools and discharge planning.
  • Mentor staff, coach best practices, and support budget and resource decisions.
  • Support plan of care conferences and rounds for optimal patient care management.
  • Identify learning needs and contribute to development of learning objectives with educators.
  • Contribute to orientation program development with Team Leader and Educator.

The salary range for this position is CAD $49.20 - $67.08 / hour

We are currently looking to fill a Relief Part-Time opportunity for a Coordinator, Home Health Service Line located in Surrey, B.C. This position is available until February 2028 or until return of incumbent.

Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Coordinator supervises the daily activities of the assigned Home Health Service Line and designated staff. Provides work direction to staff, coordinates staffing coverage, provides clinical leadership, guidance, and support to team members. Acts as a clinical resource on complex cases, facilitates and coordinates discharge planning, and assists staff to provide quality care by providing information based on specialized theory/practice, assessing clinical skills and/or assisting staff with activities as required.

Responsibilities
  1. Supervises nursing staff and coordinates the work of the clinical team on a day-to-day basis; observes work and corrects where necessary, coordinates and monitors the work flow of assigned staff, including prioritizing requests, assigns and adjusts work assignments and schedules, monitors and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
  2. Applies established criteria and prioritizes referrals and assessments; leads and facilitates actions related to discharge planning and appropriate patient placement/referral as required (e.g. after hours). Collaborates with care teams by reviewing discharge planning of patients and individual care plans to ensure optimal progression to discharge.
  3. Acts as a clinical resource by consulting with staff on complex cases and demonstrating clinical expertise; provides relevant clinical consultation and direct Home Health Service Line services as necessary.
  4. Liaises with referral sources for Home Health or community services to ensure a smooth transition to Home Health from the hospital or community.
  5. Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff in the use of standardized assessment tools and participation in discharge planning.
  6. Acts as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
  7. Initiates/leads/supports plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
  8. Identifies learning requirements of staff and, in conjunction with the Team Leader, Manager, or Educator, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
  9. Provides input into the development of a comprehensive orientation program, in collaboration with the Team Leader and Educator and other clinical staff.
  10. Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
  11. Assists the Team Leader and Manager by attending panel interviews, providing recommendations on the selection of new staff, providing feedback on staff performance, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
  12. Promotes staff awareness and involvement in research/evaluation activities and special projects and participates in research and special projects as appropriate.
  13. Collaborates with internal and external partners, participates on a variety of designated professional or operational committees as assigned.
  14. Participates in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
  15. Performs other related duties as assigned.
Qualifications

Education, Training & Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent, related clinical experience within community access and client assessment including one (1) year of supervisory experience, or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to personal vehicle for business related purposes.

Skills & Abilities

  • Clinical proficiency/expertise in gerontology, geriatrics and the needs of adults living with chronic illness.
  • Comprehensive knowledge of completing client assessments and utilizing standardized assessment tools.
  • Comprehensive knowledge of broad health care services, community access and the continuum of care.
  • Comprehensive knowledge of relevant acts, legislation, standards and policies governing access to and the operation of long term care homes, assisted living facilities and other community resources.
  • Ability to supervise and provide leadership and work direction.
  • Ability to coordinate work assignments, in collaboration with staff.
  • Ability to monitor staff workload and patient factors and adjust assignments accordingly.
  • Ability to establish priorities for services, discharge planning and overall daily operations of the service line.
  • Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice.
  • Ability to monitor, assess and address performance issues based on professional standards of practice and to provide input to the Team Leader/Manager on formal performance reviews.
  • Ability to role model, coach and mentor best practices.
  • Ability to identify individual or group learning priorities to meet identified program, organizational and learner needs, in collaboration with other clinical staff.
  • Ability to support implementation of education based on changing practices e.g. legislation and technology.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work independently and in an organized and self directed manner.
  • Ability to problem-solve and effectively deal with conflict situations.
  • Knowledge of ability to utilize applicable clinical information systems (e.g. Gold Care, Strata Pathways, Paris, Meditech ITS).
  • Ability to use Excel, Adobe Pro, Microsoft Outlook and other relevant software applications as required.
  • Ability to type 40 wpm.
  • Physical ability to perform the duties of the position.
