Job summary

The Salvation Army Booth Centre Hamilton is seeking a Case Worker to support residents experiencing homelessness or at risk of homelessness. You will conduct intake, assess needs using VI SPDAT and Diversion tools, and coordinate referrals to community resources.

Under supervision, you will provide problem-solving and case management, maintaining confidentiality and documentation, and participating in the Harm Reduction Program. This is a temporary full-time 37.5-hour role with day shifts.

Qualifications

• University/College diploma in Social Service or equivalent experience.
• Excellent oral and written communication; strong interpersonal skills.
• Ability to work with homeless or at-risk individuals and manage caseloads.

Responsibilities

• Develop and maintain a caseload of residents.
• Complete intake and assessments including VI SPDAT and Diversion tools.
• Provide counseling and referrals to services in the community.
• Coordinate with Shelter staff and other Salvation Army departments.
• Maintain documentation in log/incidents books and electronic records.
• Participate in Harm Reduction programs and safety procedures.
