Qualifications

• Active nursing license in Alberta.
• Experience in patient care or case management.
• Strong verbal and written English communication.
• Proficiency with Microsoft Office applications (Outlook/Word/Excel).

Responsibilities

• Deliver patient-centric case management within program protocols.
• Support onboarding, education, adherence, and follow-up.
• Provide disease-state and medication education to patients and providers.
• Coordinate care with physicians, clinics, pharmacies, and internal teams.
• Work with Reimbursement Specialists on funding and prior authorizations.
• Conduct telephone and virtual follow-up activities and monitor progress.
• Document patient interactions and maintain timely records in systems.
• Identify and report adverse events and product complaints.
