Shoppers Drug Mart Specialty Health Network Inc. is seeking a Neurology Nurse Field Case Manager to oversee patient and physician caseloads across a Southwestern Ontario territory, with substantial work-from-home capacity and travel.

The role emphasizes reimbursement coordination and cross-team collaboration. Ideal candidates are registered nurses with 5+ years of clinical experience, strong telephony and organizational skills, and bilingual (French/English) capabilities.

Qualifications

• RN with current, unrestricted license in the province of employment.
• BScN preferred; combination of experience and education will be considered.
• Neurology (Parkinson’s disease) experience is an asset.
• Fluently bilingual in French & English.

Responsabilités

• Manage a patient and physician/clinic caseload.
• Work from home with ~50-75% travel within territory, including overnight stays.
• Serve as primary resource for patients, physicians, health care professionals, and vendors.
• Coordinate and facilitate client progress from enrollment through therapy.
• Document thoroughly to ensure continuity of care and transitions.
• Provide reimbursement status updates and training requirements.
• Promote coordination between MDs, pharma, patients, and clinic staff.
• Deliver in-clinic, in-home, long-term care, and hospital training and build relationships.
